Transition POINTS provides parents\, siblings and other caregivers with the information and support they need to make decisions at key points across the lifespan of their loved one with ID/DD:\n\n Receiving a diagnosis and entering an early intervention program\n\n Starting school and advocating for a free and appropriate public education\n\n Preparing for and transitioning out of the school system\n\n Securing employment and a place to live outside the caregiver's home\, if desired\n\n Aging with a disability\n\n Anticipating and handling the decline and/or death of the primary caregiver.\n\n\n\nFor more details\, click here.\n\n\n\nTo RSVP\, click here.
